Accidents Will Happen

If your food program agency does not provide you with an accident report form, check with your licensing agency. Use carbon paper to make three copies so you can give one to the child’s parents and send a copy to your licensing agency. File your copy in the child’s medical folder. When you fill out the form, be sure to include the child’s name, date of incident, a brief explanation about the happenings and the action you took.
